This is a required part of the Pleasant Grove Portfolio. 

 

To create the finished copy of your Pleasant Grove Portfolio Cover Page, click on the link and follow the directions for completing and submitting this form.

 

 

 

Once you have entered the text and submitted your form an "official" finished copy of your cover page will be emailed to you.   The cover page is to be inserted into the front cover of your final Pleasant Grove Portfolio binder.  Without this form, your final portfolio will not be accepted when you go to turn it in.

 

If, after submitting, you do not receive an email, come to the VP office for assistance.

The following forms are required in your final Pleasant Grove Portfolio due on April 6 & 7, 2017Letter to the JudgesCover Page, and Best Works Log Form. It is important that you follow the directions on how to submit them and make sure that they are turned in by the due date of March 30, 2017.

Letter to the Judges

READ THE INSTRUCTIONS BELOW!!!

Minimum Length Requirement - 600 words

 

This is a required part of the Pleasant Grove Portfolio. 

 

To submit your Letter to the Judges, click on the link and follow the directions for completing and submitting your letter.

 

 

 

Once you have entered the text and submitted your form an "official" finished copy of your letter will be emailed to you. BUT NOTE that a copy of your letter is also sent to PGHS Staff for a word count verification.  IF YOUR WORD COUNT IS LESS THAN THE MINIMUM YOU WILL RECEIVE A ZERO FOR THIS ASSIGNMENT.  So, MAKE SURE that your word count is over the minimum 600 words BEFORE hitting submit!!

 

Only enter the text of your letter.  In other words, DO NOT include the normal heading items like date, address, Dear ______, etc. or normal salutation items like "Sincerely", and your name.  Both heading and salutation items will AUTOMATICALLY be included on your official copy.

 

When you receive your email open the attachment, print out six (6) copies, and sign each of the six.

One copy is for your Pleasant Grove Portfolio, another copy is for your English teacher, and the other four copies are to be submitted to your judges during your senior presentation.

 

If you do not receive an email with your completed Letter to the Judges, come to the VP office for assistance.

This form is a required part of the Pleasant Grove Portfolio. 

 

To create the finished copy of your Best Work Log Form, click on the link and follow the directions for completing and submitting this form.

 

 

 

Once you have entered the text and submitted your form, the "official" finished copy of your Best Works Log Form will be emailed to you.   That form is to be included in the appropriate place within your final Pleasant Grove Portfolio.

 

If, after submitting, you do not receive an email with the finished copy of your Best Works Log Form, come to the VP office for assistance.


 

Exemption Form

DUE DATE: FEBRUARY 10, 2017

 

READ THE INSTRUCTIONS BELOW!!!

 

This form needs to be completed by students that are missing Community Service points, OR Summative Letters because you were a student in another school during those school years.

 

To receive the completed copy of your Exemption Form, click on the link and follow the directions for completing and submitting the form to the Vice Principals’ office.

 

After printing this form, sign it and turn it into the Vice Principals’ office for processing.  If, after submitting, you do not receive an email with your filled-in Exemption Form, come to the VP office for assistance.

 

Once it has been evaluated, the form will be signed by the Portfolio Coordinator and returned back to you noting that the exemptions have been either “Approved” or “Denied”.

 

If your exemptions have been approved, keep the signed form for your records until you have verified that your exemptions have been reflected on your official Pleasant Grove Portfolio Score Sheet.
